Do you accept medicaid or medicare? 

PanCare of Florida affordable health care clinics accept both medicaid and medicare, as well as other types of medical and dental insurance coverage. Medicaid is a state program that provides health coverage for low-income individuals and families that has been expanded to cover people of all ages. Medicare is a federal program that provides health coverage for certain categories of individuals, including those with disabilities or who are aged 65 years or older. 

What if I don’t have insurance?

A sliding fee scale is available for individuals without medical or dental insurance, as well as for individuals who are underinsured. Household income and family size are used to determine the amount a patient pays for specific health care services. For instance, individuals with a lower monthly household income will pay less for any services they receive.

Is chronic care management available?

PanCare of Florida provides chronic care management for patients with two or more chronic conditions, such as chronic obstructive pulmonary disease (COPD), diabetes, hypertension, arthritis, asthma, depression, or another qualifying condition. Chronic care management can including many different services, such as personalized healthcare assistance, 24/7 access to a healthcare professional, assistance with management of medications, and help coordinating both home and community-based clinical services.
